What Your Students Need to Bring and Wear:

· Physical Exam Form (this must be signed by a parent or guardian in order for the exam to be performed.)

· $20 Registration Fee—cash or check to PORI.

· Boys wear shorts and a t-shirt.

· Girls wear shorts and a t-shirt and a backless swim suit.
